Form question types

Different question types can exist in a form. The type of question affects the way the question is scored.

Yes/No

  • Answer is either Yes or No.

  • Yes/No questions are sometimes automated. If a Yes/No question is automated, the system automatically answers the question for you when you load the form in the form fill-out area. The question is answered according to rules defined in the form.

Each answer is pre-assigned a corresponding score value, including the value Null. If N/A is provided as an optional answer, it can either be defined as Null or it can be assigned a score value (including 0).

Five Rank Scale

Answers that are selected from radio buttons, for example, Excellent, Good, Average, Fair, and Poor.

Custom Scored List

Answers are selected from a list.

Answers are sorted according to the order in which they were inserted when the form was created.

Textual answers are selected from a list.

Answers are sorted according to the order in which they were inserted when the form was created.

Answers are not assigned a score.

Numeric Range

Numeric values are entered in a text box within a predefined range of values.

The numeric answer provided represents the score of the question.

A textual answer is entered in a text box (up to 1024 characters long) and is used for data collection purposes.

Linked Form - For upgraded systems only

A linked form provides an extra layer of questions related to the target question. Clicking the question opens up a different form in a new window.

Linked forms are not stored in the Data Warehouse, and therefore, the answers submitted in the linked form are not included in reports. Only the final scores that are populated from linked forms into linked questions are included in reports.

The final score of the linked form represents the score of the question. Therefore, the maximum score for this type of question is the maximum score available for the entire linked form.
